To schedule an appointment, you may download our patient information form and send it to:

Or you may call 425-656-9627 and ask that a form be mailed to you.
We ask that you complete and return this form. You will then be contacted by a therapist, based on the information you provide. The therapist will contact you by telephone to schedule an appointment.
Occasionally we have a waiting list. Should this be the case when we receive your application, we will call to let you know when we expect to have openings.
